Care Model

Because we believe in relationship based care, we operate as either Direct Primary Care (visit fees included) or Wellness plan members simply pay a fee per visit charge.

General primary care services and office procedures with a focus on lifestyle and prevention. Wellness-based membership options for a set monthly fee. No surprises, only great care. We eliminate insurance company interference and barriers to care.

Annual Preventive Exams

Complete Physical Exams. Review preventive health and screening tests. Order/update screening as needed. Pap testing performed in office if due.

Chronic condition management

Procedures

We perform most office procedures including sutures, skin biopsies, cryotherapy, IUD, Nexplanon, pap testing, toenail removal, ear irrigation, joint injections, PRP joint injections, and more.

You do not need to be an established patient to have procedures performed.

When booking online, select appointments beginning in "New Patient" or "Established" to ensure the proper booking times. Procedure requests should be booked for an evaluation first.

We do our best to honor requests when appropriate, but the procedure may or may not be able to be performed that day.

General Medical Visits

Management of conditions such as Diabetes, hypertension, hyperlipidemia, COPD, asthma, allergies, depression, anxiety, thyroid disorders, menopause treatment, weight loss, and more.

Problem Focused Visits

Testing and treatment for new symptoms including joint pain, fatigue, mental health concerns, and more.

Evaluation and management of reproductive health, menopause symptoms, weight management evals, and more.

Sick visits or Acute care visits

Testing and treatment for respiratory illnesses, rashes, new abdominal pain/diarrhea, UTI evaluation, and more.

Treatment of minor injuries including sutures.

Wellness Plan

Monthly membership that includes our wellness plan alone. Pay for office visits on a fee per visit basis.

One-time setup fee to set up your chart, obtain your records, and get to know you and your goals. When booking online, select "Wellness or DPC Enrollment."
