Care Model

Because we believe in relationship based care, we operate as either Direct Primary Care (visit fees included) or Wellness plan members simply pay a fee per visit charge.

General Primary Care services and office procedures with a focus on lifestyle and prevention.

Annual Preventive Exams $135

Complete Physical Exams. Review preventive health and screening tests. Order/update screening as needed. Pap testing performed in office if due.

Chronic condition management $97-150

Management of conditions such as Diabetes, hypertension, hyperlipidemia, COPD, asthma, allergies, depression, anxiety, thyroid disorders, menopause treatment, weight loss, and more.

Fee per visit charges

Not ready for a monthly plan? We accept cash-based visits on a fee per visit basis when there is availability.

Problem Focused Visits $97+

Testing and treatment for new symptoms including joint pain, fatigue, mental health concerns, and more.

Evaluation and management of reproductive health, menopause symptoms, weight management evals, and more.

Procedures

We perform most office procedures including sutures, skin biopsies, cryotherapy, IUD, Nexplanon, pap testing, toenail removal, ear irrigation, joint injections, and more.

You do not need to be an established patient to have procedures performed.

Sick visits or Acute care visits $85+

Testing and treatment for respiratory illnesses, rashes, new abdominal pain/diarrhea, UTI evaluation, and more.

Treatment of minor injuries including sutures.

When booking online, select appointments beginning in, "New Patient," or, "Established," to ensure the proper booking times.

Procedure requests should be booked as an appointment titled, "Established- New Concern," and your concern will be evaluated. We do our best to honor requests when appropriate, but the procedure may or may not be able to be performed that day.

One time set up fee- allows us to set up your chart, obtain your records, and get to know you and your goals.

Once you are a member, you can also book your annual wellness lab visit or aesthetic wellness service online by selecting appointment types, "Wellness Plan member- lab," and "Wellness Plan member- service."

Wellness based functional medicine assessment

We get excited about this service! Personalized medicine at it's best! Every person should have this assessment done at least once in their lifetime, and more frequently if possible, as the insights are invaluable.

Includes initial 90 minute consult and 2- 90 minute follow up visits.

Typical testing we will perform and review: genetic testing, gut microbiome testing, organic acid testing, and metabolic testing.
