One time set up fee- allows us to set up your chart, obtain your records, and get to know you and your goals.

When booking online, select appointment titled, "Wellness or DPC Enrollment," to sign up.

Once you are a member, you can also book your annual wellness lab visit or aesthetic wellness service online by selecting appointment types, "Wellness Plan member- lab," and "Wellness Plan member- service."

Additional Wellness Plan discounts are applied in office at check out.
